Role Description
Synapse Health is seeking a detail-oriented and results-driven Implementation Specialist to support implementation operations within our Equip division. Reporting to the Manager, Implementations, this role assists with onboarding activities for new partners, services, and operational workflows across post-acute care customers—including hospice agencies, skilled nursing facilities (SNFs), and DME suppliers.
- Assist with implementation activities for new customers, partners, workflows, and operational processes within the Equip business.
- Support the execution of defined implementation plans, timelines, and tasks to ensure milestones are met.
- Assist in system setup, data entry, and configurations.
- Maintain accurate documentation of implementation tasks, dashboards, and status updates to keep the team informed.
- Track assigned action items and follow up with internal teams to ensure timely completion.
- Identify potential risks, blockers, or cross-functional gaps and escalate to Tier 2 and/or leadership when identified.
- Collaborate with cross-functional teams including SNF, Hospice, DME Operations, Product, Technology, Network, and Revenue Cycle to ensure operational readiness.
- Participate in implementation meetings and document key decisions, action items, and next steps to maintain momentum and alignment.
- Assist in maintaining organized, standardized documentation in accordance with established naming conventions and filing structures.
- Contribute feedback on workflow challenges and support continuous improvement of implementation processes.
- Help ensure all required system configurations, documentation, and operational workflows are completed prior to go-live.
- Provide post-launch support and follow-up to ensure successful transition to operational teams.
Qualifications
- High school diploma or GED required; Associate or Bachelor’s degree preferred.
- 0-2 years of experience in healthcare, DME, implementations, onboarding, operations, or project coordination roles.
- Experience coordinating cross-functional projects or operational initiatives.
- Strong organizational and time-management skills with the ability to manage multiple projects simultaneously.
- Excellent attention to detail and accuracy.
- Strong verbal and written communication skills.
- Analytical and problem-solving skills with the ability to identify risks and recommend solutions.
- Proficiency in Microsoft Office and the ability to quickly learn internal systems and tools.
Requirements
- Experience in post-acute care, hospice, SNF, or DME environments (preferred).
- Familiarity with project tracking tools such as Monday.com, Smartsheet, Jira, Salesforce or similar platforms (preferred).
- Entry/Early-Mid level Project Management (preferred).

Compensation
The pay range for this position is $24.62-30.77 per hour. This range reflects the anticipated compensation at the time of posting. Final compensation will be based on factors such as relevant experience, demonstrated skills, internal equity, and prevailing market conditions. Compensation discussions are encouraged throughout the hiring process.
