Role Description
This role involves providing Chronic Care Management (CCM) services to patients with chronic conditions. It is a phone-based, productivity-driven role where you will spend most of your workday on the phone speaking with patients, coordinating care, and documenting interactions.
- Conduct monthly CCM phone calls with patients
- Make a high volume of outbound calls daily
- Educate patients on chronic conditions and care plans
- Coordinate care with provider offices, pharmacies, and community resources
- Identify gaps in care and social determinants of health
- Create and update care plans in the EMR
- Meet established productivity and quality expectations
- Maintain patient privacy and HIPAA compliance
Qualifications
- 1–3 years of experience as a CMA/RMA, LPN, or RN
- Active certification or professional license
- Strong verbal and written communication skills in English
- Comfortable being on the phone most of the day
- Experience with EMR/EHR systems
- Organized, reliable, and able to work independently
Requirements
- Private, quiet workspace with no interruptions
- Desktop or laptop (no tablets)
- At least one monitor (dual monitors preferred)
- Noise-canceling headset
- Reliable high-speed internet (wired connection preferred)
- Satellite internet is not permitted
Benefits
- Training Pay: $21.00/hour for 4 weeks
- Post-Training Base Pay: CMA/RMA: $16.00/hour, LPN: $18.29/hour
- Monthly Incentive: $800–$1,400, based on productivity and performance metrics
